Mosaic Medical is a nonprofit clinic system, with community health centers in Prineville, Bend, and Madras. As we move towards "team based care", the RN Care Coordinator will be an integral part of a team of 2-4 providers and 2-4 Medical Assistants. This model puts patient in the center of their care and makes the RN an educator for the patient and family, a case manager for the most complicated patients and partner to the team of other health professionals. There is optional life insurance, short term disability, and accidental insurance available.Responsibilities include: •Support clinical team in a medical home model approach. •Teach patients with chronic diseases how to interface with the health care community and to grow in self-management and strategies. •Use motivational interviewing to have the patient truly engage in their own health. •Use effective communication and problem solving skills on a daily basis to provide quality patient care.
- Coordinate resources and work in a collegial relationship with elaborate health professional team. •Provide proper documentation and reporting on all patient interactions using Care Management Plus to manage patient panel.•Collaboration with healthcare team to develop toolkits and plans of care for patients and families to promote wellness and self-management.
